Multi-port valve by Waterway, found in many Cal Spas. 5 port version has 4 lateral, and one bottom port. The 4 port version has 3 lateral, and one bottom port. 3 port valve has 2 lateral, and one bottom port.
Note that all of these will have 6 screws in the top plate, with exception of the 1.5" version will have only 4 top screws.
